First School in Northeast Florida to Achieve the distinction

Elkton, Fla. (August 18, 2021) – South Woods Elementary, A Community Partnership School has become the first school in Northeast Florida to become a UCF-Certified Community Partnership School.

The Community Partnership Schools™ model at South Woods is a long-term commitment among Children’s Home Society of Florida, Flagler Health+, St. Johns County School District and St. Johns River State College. In collaboration with other local nonprofits, businesses and the faith community, core partners bring together high-quality academics, health care, counseling, mentoring and more right on-site at the school — where students and families are comfortable.

Earning the title of UCF-Certified Community Partnership School is an honor that represents years of work focused on implementing the model and changing the trajectory of students’ lives. South Woods’ UCF-Certification represents the intentional, dedicated collaboration to align necessary resources to help ensure student success and well-being. The school successfully passed all 12 rigorous benchmarks to achieve certification including governance, structure and leadership, after-school programs and comprehensive wellness services.

Since implementing the innovative Community Partnership Schools™ Model in 2017, South Woods Elementary has increased access to health care, counseling and after school activities – key needs identified by parents, teachers and community leaders in an initial assessment.

During the 2020-2021 school year, 77 percent of South Woods Elementary, A Community Partnership School students who participated afterschool tutoring showed learning gains. Impressive results at any time, but especially during a pandemic.

Additionally, the Community Partnership School opened an innovative telehealth kiosk this year — offering accessible, timely health and wellness services in partnership with Flagler Health+. The Kiosk helps connect South Woods families to primary medical care including dental, vision and mental health counseling services.
