Community Partnership Schools in Greater Pensacola

Community Partnership Schools bring together schools, families, and community partners to provide coordinated support that helps students thrive. CHS is a partner within 37 Community Partnership Schools statewide, including Pine Forest High, C.A. Weis Elementary and Bellview Middle in Pensacola.

Adoption

CHS helps children who can’t safely return home find permanent families. We work closely with child welfare and partner agencies to make sure placements are stable and supportive.

Who Qualifies:

  • Children in foster care ready for adoption.
  • Families approved and ready to adopt.

Purpose:

  • Make sure every child has a safe, permanent home.
  • Support families and coordinate with partners throughout the adoption process.

Counseling

CHS Counseling provides professional mental health support for children, teens, adults, and families facing emotional, behavioral, or life challenges. Licensed counselors work with clients in flexible settings to address concerns and strengthen coping strategies.

Who Qualifies:

  • Children, teens, adults, and families experiencing mental health or life challenges, including anxiety, depression, trauma, family conflict, grief, or behavioral concerns.
  • Services are available through in-person, school-based, or telehealth appointments, depending on location and program availability.

Purpose:

  • Provide therapeutic support to address mental health and family challenges.
  • Strengthen coping skills, family relationships, and emotional well-being.
  • Connect clients with additional resources when needed, including telepsychiatry for medication management and crisis response services where available.

Post Adoption Support

Post-Adoption Support Services provide ongoing guidance, counseling, and resources to families after adoption. Services are designed to help families navigate challenges and support healthy family relationships over time.

Purpose:

  • Support families in addressing trauma-related needs and strengthening family connections.
  • Provide counseling, including virtual counseling options, and connect families with community resources.

Healthy Families

Healthy Families is a voluntary, home‑based family support and coaching program that helps expectant parents and families of newborns build strong, nurturing foundations during the early childhood years. It’s part of the statewide Healthy Families Florida network and designed to promote positive parent‑child relationships and healthy development.

Who Qualifies:

  • Families with a prenatal referral or a newborn less than about three months old.
  • Families referred by partner agencies who would benefit from in‑home support and connection with community resources.

Who qualifies?

  • Provide supportive home visiting to strengthen parenting skills and engagement.
  • Connect families to community services and help them set and achieve family goals.
  • Support child development and help create safe, stable home environments.

Healthy Start

Healthy Start is a statewide early support and education program for pregnant women and families with newborns through age three. It’s delivered through coordinated home‑visiting and care‑coordination services to improve maternal and child health outcomes.

Who Qualifies:

  • Expecting parents and families with infants up to age three who are identified through prenatal or infant risk screening or referred by clinical and community partners.

Purpose:

  • Connect families with education, support, care coordination, and community resources that reduce risk factors for poor birth outcomes and promote healthy early childhood development.

M-Power

M-Power supports teens and young adults who are experiencing homelessness, at risk of homelessness, or transitioning out of foster care. The program provides coordinated services that help youth build the stability and skills needed for independent living.

Who Qualifies:

  • Youth and young adults ages 16–21.
  • Individuals who have exited foster care or are experiencing or at risk of homelessness.

Purpose:

  • Support the transition to independent living through housing assistance, life-skills coaching, education support, job training, and connections to health resources.
  • Provide stable, supportive environments through partnerships with community resources and host families who offer temporary housing and mentorship.

YouthWorks

YouthWorks connects teens and young adults in foster care with positive role models and career development support. The program helps participants complete their education and gain job training or placement to transition successfully into employment.

Who Qualifies:

  • Youth and young adults ages 16–24 in Escambia County.
  • Those who have dropped out or are at risk of dropping out of high school.

Purpose:

  • Provide individualized career and education planning with support from a dedicated Career Development Coach.
  • Equip youth with skills for employment, including job readiness, interview practice, and career exploration.
  • Foster connections and peer support to help participants build a positive future.

Street Solutions

Street Solutions provides outreach and support to homeless and runaway youth, helping them meet basic needs while building stability and connections to caring adults. The program also links youth to transitional housing and community resources for a successful path to self-sufficiency.

Locations Available:

Who Qualifies:

  • Youth and young adults ages 14–22 who are homeless or have run away from home.

Purpose:

  • Ensure immediate safety and access to essentials such as shelter, food, clothing, and hygiene items.
  • Connect youth with transitional living options (including M-Power Houses) and supportive community resources.
  • Provide guidance, life-skills training, and education support to promote independence and long-term stability.

Partner Note:

  • Office hours: Monday–Friday, 8 a.m.–5 p.m., call (850) 266-2700. Counselors are also available on call.
  • Partners can coordinate referrals and leverage connections with National Runaway Safeline (1-800-RUNAWAY).

Family Visitation Centers

Family Visitation Centers provide a safe, neutral setting for court-ordered supervised visitation between parents and children. The program supports healthy family interaction while prioritizing safety when families are navigating complex situations.

  • Families with court-ordered supervised visitation.
  • Cases involving safety concerns such as domestic violence, sexual assault, stalking, dating violence, or child abuse.

Purpose:

  • Provide a safe, structured environment for supervised parent-child visits.
  • Support positive parenting and the rebuilding of family relationships when appropriate.
  • Offer clinical oversight and connect families to counseling and supportive services as needed.

Wraparound (WRAP)

The WRAP program supports families at high risk of child welfare involvement, helping parents and children access services while keeping families safely together. The program emphasizes strengths-based, team-oriented support to promote stability and resilience.

Who Qualifies:

  • Children ages 0–17 and their parents.
  • Families assessed by the Department of Children and Families (DCF) as high or very high risk, with children determined to be safe at home.
  • Services are offered in Escambia County in partnership with Families First Network.

Purpose:

  • Provide coordinated, in-home case management and family support.
  • Facilitate weekly check-ins and Family Team Conferences to identify needs, set goals, and engage informal supports.
  • Strengthen family relationships, prevent further child welfare involvement, and promote positive outcomes for children and parents.

Social Services Navigator Program

The Social Services Navigator program provides on-site support in 34 Escambia County schools, helping students, families, and educators access resources for housing, counseling, employment, and emergency needs. The program connects families with services and offers guidance to navigate local systems effectively.

Teenspace

TeenSpace provides group instruction and home-visiting support for pregnant and parenting students, focusing on literacy, child development, and overall family well-being.
