Adoption

CHS helps children who can’t safely return home find permanent families. We work closely with child welfare and partner agencies to make sure placements are stable and supportive.

Who Qualifies:

  • Children in foster care ready for adoption.
  • Families approved and ready to adopt.

Purpose:

  • Make sure every child has a safe, permanent home.
  • Support families and coordinate with partners throughout the adoption process.

Post Adoption Support

Post-Adoption Support Services provide ongoing guidance, counseling, and resources to families after adoption. Services are designed to help families navigate challenges and support healthy family relationships over time.

  • Services are available to families with adopted children through age 18.

Purpose:

  • Support families in addressing trauma-related needs and strengthening family connections.
  • Provide counseling, including virtual counseling options, and connect families with community resources.
  • Offer regular check-ins and long-term support to promote stable, successful adoptions.

If you're in Palm Beach County and have children ages 0 to 8, we can provide educational and social support to help your kids succeed in school, and we can provide guidance, connections and resources to empower you to build upon your strengths as a parent.

Who qualifies?

Families with children ages 0 to 8 living in Lake Park, Lake Worth and West Palm Beach.

What to expect

In a safe, judgment-free environment, we’ll work with you to set and achieve personal and family goals so you can build your own bridge to success.
Though workshops and one-on-one support, you’ll receive valuable tips as you learn:

  • How to comfort your crying baby
  • How to teach your toddler to understand “no”
  • How to help your child manage bullying
  • Fun family activities that will help your child learn and grow
  • How to find quality, affordable childcare
  • How to access appropriate medical care if you’re pregnant
  • How to access local resources that can assist with stable housing, food and utility bills
  • And lots more

At Bridges, you’ll meet neighbors and new friends who come together to support one another through tough times and celebrate together during joyful times.

CINS/FINS

CINS/FINS provides early intervention for youth showing risky behaviors, helping them stay on track before issues escalate. We partner with schools, law enforcement, and families to make sure support is targeted and effective.

Who qualifies?

Youth referred for non-criminal behaviors or status offenses that suggest risk of deeper system involvement.

Purpose:

  • Provide alternatives to detention or formal court.
  • Connect youth and families with the right supports in the community.

Teen Outreach Program (TOP)

TOP helps teens build self-esteem, develop healthy behaviors, and gain life and leadership skills. The program encourages positive decision-making and provides a sense of purpose through group activities, skill-building, and community engagement.

  • Middle and high school students in grades 6–12 living in Palm Beach County.

Purpose:

  • Promote healthy behaviors and relationships.
  • Teach decision-making, life skills, and leadership.
  • Engage teens in meaningful peer group meetings, curriculum lessons, and community service projects to foster growth and confidence.

Safe Harbor Shelter

Safe Harbor provides 24/7 crisis support in a warm, home-like setting for youth in need of immediate safety. The program offers guidance, life-skills activities, and support to help families reunify whenever possible.

  • Youth ages 10–17 experiencing a crisis.

Purpose:

  • Offer temporary shelter, basic necessities, and structured daily activities in a safe environment.
  • Provide counseling and connect youth and families with therapy and community resources.
  • Support educational engagement and work toward family reunification when appropriate.

Community Partnership Schools bring together schools, families, and community partners to provide coordinated support that helps students thrive. CHS is a partner within 37 Community Partnership Schools statewide, including John F. Kennedy Middle in Riviera Beach and Lake Worth High in Lake Worth.
