What to expect

In a safe, judgment-free environment, we’ll work with you to set and achieve personal and family goals so you can build your own bridge to success.
Though workshops and one-on-one support, you’ll receive valuable tips as you learn:

  • How to comfort your crying baby
  • How to teach your toddler to understand “no”
  • How to help your child manage bullying
  • Fun family activities that will help your child learn and grow
  • How to find quality, affordable childcare
  • How to access appropriate medical care if you’re pregnant
  • How to access local resources that can assist with stable housing, food and utility bills

Counseling

CHS Counseling provides professional mental health support for children, teens, adults, and families facing emotional, behavioral, or life challenges. Licensed counselors work with clients in flexible settings to address concerns and strengthen coping strategies.

  • Children, teens, adults, and families experiencing mental health or life challenges, including anxiety, depression, trauma, family conflict, grief, or behavioral concerns.
  • Services are available through in-person, school-based, or telehealth appointments, depending on location and program availability.

  • Strengthen coping skills, family relationships, and emotional well-being.

Early Head Start

Early Head Start is a federally supported early childhood and family support program for expectant families and families with very young children. It focuses on healthy development, early learning, and connecting families with community supports.

Family Life Education

Family Life Education provides individualized support to help parents build confidence, strengthen family relationships, and improve parenting skills. Services are delivered in-home or in-office and are tailored to each family’s needs.

The Family Preservation & Stabilization program helps families navigate challenges such as domestic violence, homelessness, substance use, or involvement with the foster care system. Through guided support, the program strengthens family relationships and promotes stability.

Family Services Planning Team

The Family Services Planning Team coordinates mental health and behavioral support for children and families, creating individualized plans with counselors, case managers, and specialists to address each family’s unique needs.

The Family Transition program provides home-based support for families navigating life changes. Our team works alongside parents to strengthen skills, build routines, and connect families to community resources that promote long-term stability.

It’s part of the statewide Healthy Families Florida network and designed to promote positive parent‑child relationships and healthy development.

These programs help youth and young adults in or exiting foster care gain the skills and stability needed to transition successfully into adulthood. Participants receive guidance on education, employment, life skills, and housing to build productive, independent futures.

M-Power

M-Power supports teens and young adults who are experiencing homelessness, at risk of homelessness, or transitioning out of foster care. The program provides coordinated services that help youth build the stability and skills needed for independent living.

Mobile Response Team

The Mobile Response Team provides 24/7 crisis support for individuals and families experiencing mental health emergencies in Putnam County. The team offers rapid on-site intervention, safety planning, and follow-up support to help families stay safe and stable.

What to expect:

Youth will be matched with a positive adult mentor who will build self-esteem, teach the importance of respecting authority and empower them achieve educational goals.

Project 18 Mentoring: Building bridges to success for teens in foster care

Teens ages 13-21 in foster care in Palm Beach County are matched with mentors for four hours monthly over two years. Mentors provide guidance and encouragement as youth work toward high school graduation, explore college and career options, develop life skills, and learn to drive.

What to expect:

You’ll will be matched with an adult mentor in the community. You both agree to meet at least four hours per months for a full two years.

Mentors help you prepare for the next stage in your life by providing encouragement and guidance as you:

  • Work to achieve educational goals, including high school graduation
  • Visit and apply to colleges
  • Apply for jobs and gain employment skills
  • Learn important life skills, such as budgeting, cooking and finding reliable transportation
  • Learn to drive and obtain your driver license

Your mentor truly believes in your potential and is eager to empower you to strive for success.

Social Services Navigator Program

The Social Services Navigator program provides on-site support in 34 Escambia County schools, helping students, families, and educators access resources for housing, counseling, employment, and emergency needs. Street Solutions provides outreach and support to homeless and runaway youth, helping them meet basic needs while building stability and connections to caring adults. The program also links youth to transitional housing and community resources for a successful path to self-sufficiency.

Targeted Case Management

Targeted Case Management supports children with diagnosed mental health challenges and helps families navigate complex behavioral and emotional support systems. We coordinate services and connect families to community resources to promote stability at home and school.

  • A mental health diagnosis that is not a developmental challenge, or
  • Destructive behaviors that could result in self-harm or harm to others.

  • Equip families to advocate for mental health needs and manage challenging behaviors.
  • Connect children and families with counseling, behavioral therapy, evaluations, mentoring, and ongoing medical support.
  • Promote safe, stable environments so children can remain successfully at home and in school when appropriate.

Teen Outreach Program (TOP)

TOP helps teens build self-esteem, develop healthy behaviors, and gain life and leadership skills. The program encourages positive decision-making and provides a sense of purpose through group activities, skill-building, and community engagement.

The WRAP program supports families at high risk of child welfare involvement, helping parents and children access services while keeping families safely together. The program emphasizes strengths-based, team-oriented support to promote stability and resilience.

  • Facilitate weekly check-ins and Family Team Conferences to identify needs, set goals, and engage informal supports.
  • Strengthen family relationships, prevent further child welfare involvement, and promote positive outcomes for children and parents.

Wraparound Services

Wraparound Services supports children, teens, and their families in Central Florida by coordinating a network of services to address behavioral, emotional, and social challenges. The program is family-driven, youth-guided, and culturally competent, making community resources more accessible.

YouthWorks connects teens and young adults in foster care with positive role models and career development support. The program helps participants complete their education and gain job training or placement to transition successfully into employment.
